The Care Coordinator II for ChristianaCare supports the Heart Failure multidisciplinary team maintain the required core measures for the hospital's heart failure accreditation.  This hybrid position will work have a rotating schedule depending on the needs of the Heart Failure team by either working remotely to complete post heart failure discharge follow-up phone calls or working on-site at ChristianaCare as an inpatient nurse navigator with a focus on face-to-face patient education and discharge planning.  
The successful candidate will collaborate with patients, caregivers and members of the multidisciplinary care team to develop a patient centered plan of care to meet identified care goals while promoting patient wellness and autonomy through advocacy, communication, education, and identification and referral to community resources or other case management programs.

Education & Requirements:

• Current RN license.
• BSN required, MSN preferred.
• BLS Certification is required.
• Minimum of 1-2 years of clinical experience in acute care.
• Prior Cardiology/Heart Failure experience preferred.
• Case management of utilization management experience preferred.
• Case management Certification (CCM) required within 18 months of eligibility.
